Resume
The aim of the study was to improve the results of treatment of patients with BPH by an individualized approach to choosing the method of endoscopic treatment HOLEP or TUR, depending on prognostic factors affecting the course and outcome of the disease. Materials and methods of the study: 335 patients who underwent examination and inpatient treatment at the Urological Complex clinic from 2016 to 2020 were included in the retrospective study. The diagnosis of benign prostatic hyperplasia was established on the basis of: complaints, anamnestic data, a set of laboratory and instrumental research methods, the scope and methods of the studies are described in the corresponding chapter below. In all patients in the postoperative period, the diagnosis was confirmed histologically, all cases of malignant formation during histological examination in the postoperative period were excluded from the study. Results of the study: A comparative analysis of the effects of obesity revealed a direct correlation between the degree of obesity and the frequency of long-term complications. The more severe the degree of obesity, the longer the rehabilitation period of these patients, the duration of surgery, the duration of catheterization, the volume of blood loss, as well as hospital stay increases. Thus, during the TUR and HOLEP, the frequency of long-term complications with a gland volume of less than 40 cm3 turned out to be identical, with a gland volume of more than 70 cm3, the frequency of long-term complications in the HoLEP group was 5.4%, and with TURP 9%.
