MORPHOLOGICAL CHANGES IN THE KIDNEYS DURING CHEMOTHERAPY

Kidney damage is a common and serious complication of cancer and anticancer therapy. Its spectrum is very broad and causes damage to the glomeruli, affecting the renal tissue. In addition, the risks associated with complications after bone marrow transplantation (BMT), infections (including sepsis) due to immunosuppression, and tumor lysis syndrome should be considered. Chemotherapy drugs often lead to the development of acute kidney injury. This article discusses the risk factors for kidney damage in cancer patients. They are divided into two groups. General, systemic ones include infiltration of the kidney tissue with tumor cells, dysproteinemia and electrolyte disturbances, as well as a decrease in circulating blood volume. Local (renal) risk factors include microcirculation disorders, drug biotransformation, a high concentration of nephrotoxic substances in the proximal tubules, and the latter’s sensitivity to ischemia. The need for long-term use of high doses of a combination of cytostatics is manifested by the formation of intratubular deposits due to a direct cytotoxic effect and poor solubility of not only chemotherapeutic agents, but also their metabolites. Early diagnosis, timely prevention and treatment of these complications significantly improve non-oncological outcomes of patients’ treatment.
