EVALUATION OF THE RESULTS OF EXTRAPERITONEAL RADICAL CYSTECTOMY

To evaluate the immediate results of radical cystectomy performed by extraperitoneal access in patients with invasive bladder cancer.79 patients with invasive bladder cancer stage T2-3N0-2M0, aged 63.2±4.3 years, were examined. Extraperitoneal access according to the technique developed by us with a standard dissection of the pelvic lymph nodes. Systematization of postoperative complications according to the Clavien-Dindo classification showed that mild complications (grades I and II) were observed in 8.9% of cases, grade III – in 11.4%, grade IV – in 6.3% and grade V – in 5.1%. The average life expectancy of patients after surgery was 71.6 months, local recurrence developed in 4 (2.2%) patients, and distant metastases subsequently developed in 13 (7.2%) patients. Radical cystectomy performed by extraperitoneal access is a safe and reliable surgical approach that has certain functional advantages in terms of early restoration of intestinal motility in the postoperative period, which helps to reduce the incidence of intestinal obstruction and, associated with it, the need for repeated operations.
