ATYPICAL CLINICAL COURSE OF NECROTISIUS FASCIITIS IN THE BACKGROUND OF DIABETES MELLITUS

Resume

Purpose: to study the peculiarity of the clinical criteria for necrotizing fasciitis in the presence of diabetes mellitus. Materials and methods: the basis of this work is the analysis of the treatment of 23 patients with necrotizing fasciitis for the period from 2019 to 2022 in the Department of Purulent Surgery and Surgical Complications of Diabetes Mellitus of the Multidisciplinary Clinic of the Tashkent Medical Academy and consideration of the general issues of this pathology outlined in domestic and foreign medical literature of recent years. Аttention was drawn to the publication of T. Simonart et al. (2004), in which the authors report on the importance of determining the activity of serum creatine phosphokinase (a muscle tissue enzyme) in the early diagnosis of necrotizing fasciitis. In the presence of fascial necrosis, there is almost always a reaction of the underlying muscle tissue, which determines the increase in the level of creatine phosphokinase (CPK) activity. Results: the average duration of the course of necrotizing fasciitis in patients with diabetes mellitus was studied. In addition, the localization of the process and the determination of CPK activity were studied. Conclusion: The development of optimal clinical and laboratory criteria for the diagnosis of necrotizing fasciitis in patients with diabetes mellitus is a priority in solving this problem.
