Resume
An increase in the number of kidney diseases among children, on the basis of which the increase in the number of their complicated forms leads to an increase in the number of nephrureterectomies. Despite the fact that the necessary examination methods have been developed to identify congenital and acquired kidney diseases, the delay in their early diagnosis leads to the complications of kidney diseases, and as a result, the quality of life of patients decreases. In particular, the problem of developing chronic kidney failure in children left with only one kidney remains urgent (M.A. Lopatkin, A.G. Martov, 2004). Currently, the number of operations performed on kidneys is 14-27% of surgical interventions performed in urology (L.A. Deryugina, 1991).
According to some authors, the failure of the single kidney in a child is the result of a gradual decrease in the compensatory ability of a healthy kidney after receiving a high load (contralateral kidney activity) (Dutov V.V., 1980; Hartshorne N., 1991; Orlova G.M., 2002 , Shokeir AA, 2003). According to a number of other scientists, one of the most important factors affecting the preservation of single kidney activity is the addition of the microbial inflammatory process (Laurent O. B., 1999).
The authors suggest that nephroureterectomy should be performed only when there is absolute certainty that the function of the contralateral kidney is preserved. However, it has been proven that the morphological changes in the kidney preserved after nephroureterectomy are similar to the changes in the organ removed during the operation. Evaluation of the morphological signs of pyelonephritis and hydronephrosis in children showed that renal dysplasia is more common in children with a specific pathological process. However, the authors did not examine the relationship between the functional status of the single remaining kidney after nephroureterectomy and the degree of dysplasia of the removed organ tissue.
