OPTIMIZATION OF SURGICAL TREATMENT METHODS FOR MULTIPLE AND RECURRENT FORMS OF LIVER ECHINOCOCCUS

Resume

The results of surgical treatment of 67 patients with multiple and recurrent echinococcosis of the liver were analyzed. 46 patients had primary multiple liver echinococcosis (MLE). Recurrent forms of EP were observed in 18 patients. Before the operation, the cyst burst into the biliary tract in 22% of cases, into the pleural cavity in 11% and 5.6% into the abdominal cavity. The clinic of obstructive jaundice was detected in 28%, cyst suppuration in 22% of patients. In the treatment of MLE, taking into account the localization of cysts and the nature of complications, the following operations were used: ideal echinococcectomy in 17.4%, cystpericystectomy in 17.4%, echinococcectomy in combination with cystperecystectomy in 13%, polyclosed echinococcectomy in 52.2% of patients. Endoscopic papilosphincterotomy with external drainage was performed in patients with complicated EP with the development of obstructive jaundice. Analysis of the results of surgical treatment of MLE and recurrent EL showed that the most effective method of surgical treatment is ideal echinococcectomy. According to the indications, external drainage of the residual cavity (RC) and biliary tract was performed, which is the method of choice in the treatment of patients with MLE and RLE. Echinococcectomy of the liver from a mini access creates favorable conditions for strict adherence to the principles of aparasiticity and antiparasiticity.
