Relevance. The pathogenesis of chronic HBV infection is based on the interaction of the host immune system with a multiplying virus. It has been established that chronic viral liver lesions are an immuno-mediated condition that depends on the replicative activity of the virus and the reactions of the immune system to this virus. And under the influence of pesticides, various stages of immunoreactivity, nonspecific factors of the body's defense are suppressed, autoimmune restructuring of the body is caused The aim. To study the molecular genetic and immunological features of the course of viral hepatitis B in individuals who had different contact with pesticides. Materials and methods. We divided all 169 patients examined into 2 groups depending on the presence of contact with pesticides. The main group consisted of 98 patients with viral hepatitis B who have been living and working in the cotton-growing area for the past five years without leaving. The comparative group consisted of 71 patients with viral hepatitis B who were exposed to less pesticides. The control group consisted of 30 practically healthy people. Results and conclusion. In a comparative study of groups of patients with HCV, depending on the degree of exposure to pesticides on their body, it was revealed that the HBV virus infection in the group of patients who have lived under the influence of pesticides for a long time is characterized by significantly more frequent (2.4 times) detection of a mutant (HBeAd-negative) strain. On the part of the immunoreactivity of the body, these patients were characterized by some suppression of both the number of leukocytes and the number of lymphocytic cells, and with a suppressed T-cell link of the immune system (CD3+, CD4+, CD8+), there was a significant activation of the B-link, manifested in increased activity of class M immunoglobulins and a high level of immunocomplex processes with a weakly expressed antiviral response.
