Resume
To analyze first-hand experience in treating newborns with spontaneous gastrointestinal perforations and develop an optimal algorithm for diagnosis and disease management to improve treatment outcomes for this group of patients.The work is based on the results of examination and treatment of 50 newborns with perforated peritonitis, among which 38 patients had necrotizing enterocolitis, and 13 had spontaneous gastrointestinal perforations. Based on a clinical study of newborns, clinical features of spontaneous gastrointestinal perforations compared with intestinal perforations in necrotizing enterocolitis have been identified. The most significant risk factors for spontaneous perforations of the gastrointestinal tract in newborns were acute birth asphyxia and pathology of the respiratory system, which required tracheal intubation in 100% of children. Respiratory distress syndrome was a leading risk factor in spontaneous perforations. Surgical tactics was determined by the type of perforation, the level of localization of the perforated hole, the prevalence of the pathological process in the wall of the hollow organ, and the overall health status of the child. In spontaneous perforations, the operation of choice was closure of perforation in 11 children. In 2 patients with spontaneous perforations, direct interintestinal anastomoses were performed with large defects in the intestinal wall, a limited pathological process, and the absence of widespread peritonitis. Case mortality rate was 50% among newborns with perforated necrotizing enterocolitis and 26% among patients with spontaneous perforations. Among children who died, the vast majority were premature newborns with low and very low body weight and serious comorbidities.
