INFLUENCE OF VITAMIN D ON THE COURSE OF ATOPIC DERMATITIS

Resume

The aim of the study was to determine the role of parasites and Vitamin D (VD) level in AD course with the development of appropriate treatment. Parasitological diagnosis includes triple coproscopy with 2-3 days interval of 2-3 days. In cases when parasites were not detected, the concentration method of I.S. Ritchii et al. modified by Truant et al. (1981) was usedю Total IgE level and VD were detected with commercial ELISA kits. It was found that intestinal parasites endemic to Uzbekistan A. lumbricoides, E. vermicularis, H nana, G. lamblia stimulate the development of AD, aggravating the course and significantly increasing the level of IgE, all patients with parasitosis examined by us consulted a doctor because of exacerbation of AD. which contradicts the hygienic theory of Strachan D. (2000) about suppression of allergic and inflammatory processes by helminths, because all patients examined by us went to a doctor because of exacerbation. Determination of VD level in parasite-free AD patients with characterisation of severity of course according to SCORAD scale and the level of total serum IgE showed that in AD patients without concomitant parasitosis and with VD insufficiency/deficiency the highest IgE level and more severe course were determined in VD-deficient patients, which is consistent with the data of Bastyte D, et al (2024) on the association of AD and VD level reduction. Apparently, VD reduction may be associated with polymorphisms of VD receptor and VD binding protein. In addition, increased permeability of the intestinal barrier, changes in immune response, composition of the gut and skin microbiome may play a role [2;3]. 9 patients with severe AD aged 7 to 14 have VD deficiency, 3 have VD deficiency. Serum total IgE levels were dramatically elevated. The patients received 1000 IU o0f VD for 30 days. After the course of treatment, improved SCORAD score ameliorateded and IgE decreased In two patients with staphylococcal infection the area and intensity of the lesions decreased, apparently due to stimulation of antimicrobial peptide production.
