The number of taken medications has been repeatedly shown to be an important factor in treatment propensity. It has been proven that adherence to treatment is 30% when taking 3 drugs, 40% when taking 4, 50% when taking 5 or more, and even 100% abstinence. Purpose of the study: to study the effectiveness of treatment compliance in patients with uncontrolled arterial hypertension (AH) who received triple antihypertensive therapy in a fixed and free combination. Material and methods: The study included 86 patients with grade 1-3 hypertension. The average age was 63.1±8.9 years. In accordance with the clinical protocol, we assessed the MMAS-8 questionnaire “8-item Morisky Medication Adherence Scale” to assess the propensity to treatment of patients. Patients received three-component antihypertensive therapy (AHT) in fixed group 1 (n=44) and group 2 (n=42) in a free combination. Results: According to our study, 10 (11.9%) patients were under 44 years old, 36 (42.8%) patients were 45-59 years old, 38 (45.2%) were 60 - 74-year-old patients. Intergroup analysis revealed no statistical differences in age between groups 1 and 2 before treatment. Before treatment, regardless of the age of the patients, on average 70% of patients had a low adherence for treatment (<5 points). At the end of follow-up, the MMAS-8 test was repeated in both groups of patients. More than 80% of patients achieved target blood pressure levels during the 24-week triple AHT. At the same time, in the young group, adherence increased by only 10%, which is significantly lower than in the middle-aged and elderly group, where adherence to therapy increased 2 or more times compared to the young. According to the results of treatment, adherence to therapy significantly increased in 1st group: 8 points in 19 (41.3%) patients, 6-7 points in 14 (30.4%), 5 or less points in 13 (28.2%) patients. In 2nd group, achieving high adherence was 2 times less: 7 (17.5%) patients scored 8 points, 7 (17.5%) scored 6-7 points, 26 (65%) patients scored ≤5 points, which is 2.5 times more than in 1st group. Conclusions: Triple AHT for 24 weeks demonstrated increased patient’s adherence, with a significant increase in adherence in middle-aged and elderly patients compared with younger patients. Patients receiving a fixed combination of 3 drugs had a significant increase in adherence to treatment compared to patients receiving a free combination.
