Resume

Hip replacement is currently considered one of the most common operations on large joints. Trends in the development of endoprosthetics make it possible to take into account errors and understand the prospects and development paths of this branch of surgery. Target. Analysis of trends in the development of hip replacement. Materials and methods. To achieve the goal, a search and analysis of published works on the topic of hip arthroplasty was carried out in the bibliographic databases Elibrery, Pubmed and Web of Science, including studies examining the main difficulties, successes and achievements in the field of hip arthroplasty. Results. The history of endoprosthetics, difficulties and overcoming them are considered, the role of Russian and foreign researchers in the formation of modern foundations of hip replacement is shown. Discussion. In recent years, hip replacement has become less invasive, with better materials that are more wear-resistant and more biocompatible, and complication prevention and perioperative care have improved significantly along with advances in pain control, anesthesia, and rehabilitation. The conditions for the durability of the prosthesis, formulated by Charnley (1970), remain relevant today: the required range of movements with minimal energy consumption; reliable fastening of the prosthesis to the bones, low coefficient of rotational friction between parts of the prosthesis; The strength of the prosthesis must be sufficient for long-term operation without damage under load, and when worn, it must be possible to replace it. Conclusion. The ultimate goal of hip surgery is and remains to relieve pain and restore the patient's mobility. The future of hip replacement may involve personalized implantology, patient-specific implants and AI-assisted surgery, as well as robotics.
