Resume
Gastric and duodenal ulcer bleeding in patients with liver cirrhosis represents a complex clinical issue due to coagulopathy, portal hypertension, and impaired liver function. The use of combined endoscopic hemostasis methods, including adrenaline injection and vessel clipping, along with prophylactic antibiotic therapy, has proven effective in reducing recurrence rates and mortality. In the main group, the recurrence of bleeding was 5%, and mortality was also 5%, significantly lower compared to the control group (recurrences – 21%, mortality – 18.4%). The primary risk factors for complications and death were the severity of cirrhosis (as classified by the Child-Pugh score) and infectious complications such as spontaneous bacterial peritonitis and pneumonia. Bleeding from gastric and duodenal ulcers in patients with liver cirrhosis and portal hypertension requires a comprehensive approach, including prophylactic endoscopic ligation of varicose veins.
