THE ROLE OF STABILITY OF OSTEOSYNTHESIS USING THE ILIZAROV APPARATUS FOR FUSION OF PSEUDARTHROSIS OF THE EXTERNAL CONDYLUS OF THE HUMERUS IN CHILDREN

Resume

In 2009-2023, the results of surgical treatment of 55 patients (5 adults, 50 children) with pseudoarthrosis of the external condylus of the humerus were studied using bone grafting technique with autografts from the diaphysis of the patient's own fibula. There were 33 male and 22 female representatives. The age of the patients ranged from 5 to 36 years, the duration of the injury ranged from 6 months to 27 years. 50 patients had contracture in the joint: range of motion up to 30 degrees in 13 patients, up to 50 degrees in 11, up to 70 degrees in 12, up to 90 degrees in 5, up to 120 degrees. – in 9. In 27 patients, valgus deviation of the forearm was observed: among them, in 11 patients, valgus did not exceed 10 degrees, in 10 patients, 15-25 degrees of valgus of the joint was observed, in 6 patients, severe valgus – 30-40 degrees. Cubitus valgus developed as a result of fragment displacement outward + anterior + proximally and as a result of bone tissue resorption of the epiphysis and metaphysis of the humerus. Severe Cubitus valgus was usually accompanied by late neuritis of the ulnar nerve in 6 patients. In 12 patients who underwent bone grafting with the Ilizarov apparatus, fusion was achieved in 11 (99.17%). Of 30 operations with bone grafting, pin fixation and external immobilization with a plaster cast, fusion was achieved in 24 (80%). In 6 (20%) patients, fusion of the grafts was achieved on the metaphysis side, and on the side of the NKPK fragment, weak consolidation was noted, and radiographically non-union. After using the closed application of the Ilizarov apparatus, fusion of the pseudoarthrosis was achieved in 5 patients.
