The aim of the study. To predict and prevent obstetric hemorrhage by examining early hemocoagulation markers in the third trimester of gestation and their role in evaluating the effectiveness of corrective therapy. Materials and methods. A survey was conducted on 170 pregnant women. The main group was divided into 2 groups and 2 subgroups: I - control group, pregnant women with a physiological course of pregnancy; Group II comprised 110 pregnant women with a high risk of bleeding during childbirth. The 2nd group was subdivided into 2 subgroups: II-A pregnant women who received traditional therapy; II-B complex therapy included anticoagulant - clexane. Results obtained. The study demonstrated the importance of determining fibrinogen levels, which, in combination with hemogram and hemostasiogram parameters, allows for the assessment of the level and state of hemostasis disorders in pregnant women, enabling the prescription of adequate treatment. The applied treatment methods (traditional and those proposed by us) had different effects on coagulation hemostasis parameters (hemostasiogram). The effectiveness of treatment in the comparison group of pregnant women was 28.6%, while in the main group of pregnant women it was 85.7%, indicating the high effectiveness of the treatment we proposed. Conclusion. An increase in coagulation potential, reflected by more than a 2-fold rise in fibrinogen and D-dimer concentrations in citrated plasma, should be interpreted as a prognostic marker for the development of bleeding. Determining D-dimer levels in pregnant women at risk of bleeding during childbirth allows for the optimization of adequate differentiated prevention of thrombohemorrhagic complications and contributes to improving the outcomes of deliveries.
