Resume
Objective: To study bioimpedancemetry data and their association with renal function biomarkers, as well as clinical, metabolic, endocrinological and immunoinflammatory parameters in various obesity phenotypes. Materials and methods: The study was carried out with the participation of the international joint grant project "BERNICA" (Enhancing the educational and research capacity in the field of nutrition and dietetics in Central Asian countries) of the European Union's international Erasmus+ fund. The study involved 224 Uzbek women of childbearing age who were registered in Tashkent city family clinics in 2022-2023 and were diagnosed with obesity, who were divided into two cohorts: the first cohort included 133 women with metabolically complicated obesity (mean age 42.0±0.5 years), and the second cohort consisted of 91 women with metabolically healthy obesity (mean age 41.7±0.7 years). The control group included 45 healthy female volunteers (mean age 43.2±0.8 years, body mass index <30 kg/m², waist circumference <80 cm). The groups were assessed for anthropometric parameters, blood pressure, bioimpedance analysis, biochemical and lipid profiles of the blood, levels of leptin, insulin, cystatin C and uromodulin in the blood serum, as well as gradations of microalbuminuria in the urine. The glomerular filtration rate for cystatin C and creatinine was calculated, and the obtained data were compared. Results: In both obesity cohorts, the level of microalbuminuria and cystatin C was statistically significantly higher in the first cohort compared with the second and control groups, while the concentration of uromodulin in the blood serum was lower than in the second and control groups. The dynamics of these biomarkers correlated with a decrease in the estimated glomerular filtration rate, especially in the first cohort. Also, an increase in the body mass index was associated with an increase in cardiometabolic disorders, an increase in the gradation of microalbuminuria, a decrease in the level of uromodulin in the blood serum and an increase in cytokinemia (p < 0.001). Conclusion: In both obesity cohorts, an increase in the degree of obesity is accompanied by an increase in cardiometabolic risk and an increase in subclinical kidney damage. It seems appropriate to determine the levels of uromodulin and cystatin C in the blood serum, as well as an assessment of the gradation of microalbuminuria for the early diagnosis of renal dysfunction in obesity.
