Objective: To assess left ventricular (LV) diastolic function and global longitudinal strain (GLS) in patients with ischemic heart disease (IHD) and type 2 diabetes mellitus (T2DM). Materials and Methods: The study included 98 patients with IHD (mean age 53.5±8.1 years). They were divided into two groups: Patients with IHD and T2DM (n=52) and Patients with IHD without T2DM (n=46). All patients had preserved systolic LV function (LV ejection fraction >50%). Diastolic function was assessed using transthoracic echocardiography (E/A, e', E/e', LAVI). GLS was analyzed using the speckle tracking echocardiography method. Results: Patients with IHD and T2DM showed more pronounced LV diastolic dysfunction (E/e' = 14.7±0.14 vs. 13.6±0.12 in the non-T2DM group). GLS values were significantly lower in patients with IHD and T2DM (-14.8% vs. -16.5%, p<0.05). Correlation analysis revealed a negative association between the degree of diastolic dysfunction and GLS values (r = -0.65). Conclusion:The combination of IHD and T2DM exacerbates LV dysfunction, reducing longitudinal myocardial deformation and impairing diastolic function. These findings emphasize the need for early detection of subclinical myocardial dysfunction using modern echocardiographic techniques to optimize patient management.
