Resume
This article examines the clinical and psychological characteristics of patients diagnosed with severe chronic kidney failure (CKD) undergoing scheduled hemodialysis. The study included 80 patients at different stages of CKD who were receiving dialysis at a specialized nephrology center. The authors conducted a comprehensive clinical-psychological assessment, which involved the analysis of character accentuations (based on the Leonhard–Schmieschek method), levels of anxiety (using the Spielberger–Khanin scale), psychological defense mechanisms (using the Plutchik–Kellerman method), and coping strategies. The results revealed that patients with dysthymic, emotive, and sensitive personality accentuations more frequently exhibited pronounced anxiety, depressive reactions, and reduced psychological flexibility, particularly at stages 4 and 5 of CKD. The most common defense mechanisms included denial, regression, repression, and compensation, whose intensity increased with the worsening of the clinical condition. A link was also found between a decline in quality of life and the use of maladaptive coping strategies. The findings highlight the importance of preventive and corrective psychological interventions for this patient group, emphasizing the need for an interdisciplinary approach.
