Acute appendicitis (OA) is one of the urgent surgical pathologies, and its course against the background of cirrhosis of the liver (CP) presents significant diagnostic and therapeutic difficulties. Immunodeficiency, blood clotting disorders, ascites, and portal hypertension associated with CP alter the clinical manifestations of OA, making it difficult to diagnose and choose effective treatment tactics in a timely manner. The study was conducted in the Bukhara branch of the Russian Academy of Medical Sciences in the period 2017-2024 with the participation of 64 patients with OA on the background of CP. They were divided into two groups: 24 patients in the retrospective control group and 40 patients in the prospective study group. Informative value of laboratory markers of inflammation (C-reactive protein, leukocytes) and instrumental diagnostic methods (ultrasound and magnetic resonance imaging) it was appreciated. It has been confirmed that leukopenia in patients with OA on the background of CP is associated with impaired immune system, hypersplenism, or metabolic liver dysfunction, especially in the decompensation stage. According to the results of the study, leukocytosis is not a reliable marker of OA, whereas C-reactive protein (CRP) has shown high diagnostic significance. The effectiveness of using the AIR (Appendicitis Inflammatory Response) and AAS (Adult Appendicitis Score) scales for the diagnosis of OA in patients with CP has also been confirmed. MRI showed higher sensitivity and specificity compared to ultrasound in patients with ascites. In the main group, 8 (20%) patients were successfully treated using the NOM (Non-Operational Management) strategy, which confirmed the effectiveness of the conservative approach in certain cases.
