Resume
Total knee joint endoprosthetics (TKA) remains the "gold standard" for treatment of severe gonarthrosis, significantly reducing pain syndrome and ensuring stable long-term results. However, the recovery of patients after surgery is variable in nature, and most of them retain muscle weakness of the lower extremities and functional limitations compared to the control groups of comparable age. The purpose of this review is to systematically analyze scientific publications, including controlled and randomized studies devoted to the effectiveness of outpatient rehabilitation after TKA in both short-term and long-term perspectives. Special attention was paid to preoperative preparation and postoperative recovery, as well as the role of physical activity recommendations in the overall rehabilitation strategy. The following interventions used in clinical practice after TKA are considered: • preoperative training and therapeutic physical training, • Hardware Passive Mobilization (CPM), • muscle strengthening exercises, • hydrokinesotherapy, • balance training, • use of a tourniquet during intervention, • Positioning and type of implants, • mobile apps and wearable devices, • differences in postoperative protocols, • use of orthoses, • neuromuscular electrostimulation • conditions for rehabilitation. Intensive strength and functional exercises for patients over 45 years of age, performed in a gym or in water, with a gradual increase in load depending on the recovery dynamics, proved to be most effective in improving functional indicators and quality of life. Taking into account the individual approach to the patient, optimal long-term results are achieved when undergoing outpatient physiotherapy under clinical conditions under the supervision of a specialist. The review also highlights the impact of rehabilitation methods on the quality of life and overall well-being of patients who underwent total knee joint endoprosthetics, including at the clinics of the Samarkand State Medical University.
