Resume
Severe psoriasis requires systemic therapy, and biologic agents targeting key pro-inflammatory cytokines (TNF-α, IL-17, IL-23) have demonstrated high clinical efficacy and a favorable safety profile. This comparative study evaluated the efficacy of three biologic agents, adalimumab, secukinumab, and guselkumab, in 90 patients with severe plaque psoriasis. The degree of repigmentation was monitored for 16 weeks using the PASI scale (75, 90 and 100), as well as the dynamics of quality of life using the DLQI. The data showed that secukinumab provided the most rapid early symptom reduction, while guselkumab demonstrated the best rates of sustained remission and complete skin repigmentation. The side effect profile in all groups was comparable and did not require withdrawal of therapy. The results underscore the need for a personalized approach to the selection of a biologic agent, taking into account disease severity, clinical response rate and individual tolerability
