The pharmacodynamic characteristics of typical (classical) neuroleptics are determined by their specific neurochemical mechanism of action, which consists in the ability to selectively suppress do-pamine receptors of the second type (D2 receptors) localized in various functional areas of the brain, including the striatum, the nucleus accumbens, and the cerebral cortex. The pathogenesis of these side effects is directly related to the blockade of D2 receptors in the nigrostriatal system, which caus-es a violation of physiological dopaminergic regulation in this area. The side effects of psychotropic drugs (typical antipsychotics) in the form of extrapyramidal disorders remain the most important, urgent problem in the treatment of psychosis. The use of validated psychopathological assessment tools, such as ESRS and PANSS, makes it possible to increase diagnostic accuracy, timely adapt therapy, and improve the prognosis of extrapyramidal disorders when using antipsychotics. The purpose of this study was to study the clinical course, types, and severity of extrapyramidal disorders in patients receiving antipsychotic (AP) therapy. Materials and methods. The study included 60 patients (30 men and 30 women) aged 18 to 65 еars, hospitalized with a diagnosis of schizophrenia (ICD-10: F20), who were treated with typical antipsychotics (haloperidol, chlorpromazine, trifluoperazine), according to the principles of com-bined and monotherapy. Results. The use of validated assessment tools such as ESRS and PANSS has made it possible to improve the accuracy of diagnosis at the early stages of the development of extrapyramidal disorders, adapt therapy in a timely manner, and improve the prognosis of worsening clinical disease. Based on the results of the study, we were convinced that extrapyramidal disorders (EPD) are a common side effect of typical antipsychotics, and occur in more than 70% of patients. The most common forms of EPR are parkinsonism and akathisia. The average score on the ESRS scale was 17.2 ± 6.4, reflect-ing the average severity of extrapyramidal symptoms. A positive correlation was found between the severity of EPR and the psychopathological state (PANSS), however, with an improvement in mental state as the dose of antipsychotics increased, an increase in some extrapyramidal symptoms was not-ed. Conclusions: The findings emphasize the need for a comprehensive approach to the treatment and monitoring of patients receiving antipsychotic therapy, with regular assessment of side effects. The use of the ESRS and PANSS scales can be effective for objectifying both the therapeutic effect and the side effects of AP, which contributes to the individualization of pharmacotherapy
