DIFFERENTIAL DIAGNOSIS OF BRONCHIAL ASTHMA WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASES

Resume

Differential diagnosis of bronchial asthma and chronic obstructive pulmonary disease (COPD) is a major clinical challenge, especially in the context of polymorbidity and increasing prevalence of chronic respiratory diseases. Despite the similarity of symptoms such as chronic cough, dyspnea, wheezing episodes, bronchial asthma and COPD differ significantly in etiopathogenesis, age of onset, risk factors, nature of inflammation and reversibility of bronchial obstruction. In the present work, the key clinical, functional and laboratory parameters that allow effective differentiation of these nosologies are considered. Special attention is paid to spirometric criteria, bronchodilation test, eosinophil and IgE levels, as well as the significance of smoking history and allergic history. The presented data support the need for a comprehensive and personalized approach to diagnosis, as well as the distinction of patients with a crossover phenotype - asthma-COPD crossover syndrome (ACOS), for whom combined therapy is required.
