Resume

Differentiated thyroid carcinomas are assessed differently in terms of prognosis in European, Japanese, and American clinical guidelines, which leads to differences in approaches to their treatment. Materials and methods. The data of 152 patients with papillary thyroid carcinoma who underwent surgical treatment between 2008 and 2023 were analyzed. The average follow-up period was 15.4 years, with a maximum of 28 years. Results. The study showed a favorable prognosis for the disease, despite the lack of sonography and fine-needle biopsy during the years under consideration, which led to a later diagnosis and surgical treatment of the tumor process. The key factors affecting the prognosis were the size of the tumor at the time of surgery and the presence of distant metastases. The long-term results of treatment were almost independent of the extent of surgical intervention (total thyroidectomy or partial removal of the gland) and the performance of prophylactic lymphodissection. Mortality rates and the frequency of relapse in patients who did not receive therapy with radioactive iodine corresponded to the data presented in the literature
