This study analyzed the clinical effectiveness of endoscopic vacuum therapy (EVT) in the management of duodenal perforations and injuries treated between 2021 and 2025 at the Bukhara and Khorezm branches of the Republican Scientific Center for Emergency Medical Care, as well as their emergency surgery departments. Closed duodenal injuries accounted for 1.9% of all abdominal organ injuries. In most patients, EVT was performed in combination with an upper midline laparotomy and Kocher–Clermont mobilization. Under negative pressure, EVT enabled infection control, stimulation of granulation tissue growth, and acceleration of healing. The use of EVT resulted in reduced complication rates, fewer relaparotomies, shorter hospital stays, and higher primary healing rates. The findings confirmed the effectiveness and safety of EVT in severe duodenal injuries and suture dehiscence, supporting its broad implementation in clinical practice.
