ETIOLOGY AND PATHOGENESIS OF ROTATOR CUFF TEARS

Rotator cuff tears are among the most common causes of shoulder joint dysfunction and often lead to a significant decline in patients' quality of life. These conditions frequently require not only conservative treatment but also complex surgical intervention. The development of degenerative changes in tendon structures is caused by the interplay of multiple internal and external factors. Internal factors include age-related degeneration, reduced tissue vascularization, chronic inflammation, tenocyte apoptosis, and increased oxidative stress. External factors involve subacromial impingement, anatomical variations of the shoulder girdle, and excessive mechanical loading, particularly among individuals in specific professions or athletes. This article summarizes and analyzes current scientific evidence on the pathophysiological and molecular-cellular mechanisms underlying rotator cuff degeneration. It highlights the potential for applying bioengineering technologies, regenerative medicine, cell therapy, and genetic engineering in the development of novel approaches for the prevention and treatment of these pathologies.
