The aim of this study was to evaluate the efficacy and safety of a novel technique for the treatment of central defects of the anterior vaginal wall using a vascularized flap. This prospective study included patients with a confirmed diagnosis who underwent organ-preserving reconstruction with preservation of tissue blood supply. Outcomes were assessed using objective anatomical criteria (POP-Q) and subjective quality-of-life measures (PFIQ-7, PFDI-20) before and after surgery. At 6 months of follow-up, the majority of patients achieved complete anatomical restoration and a marked reduction in symptom severity. Quality-of-life scores improved significantly, reflecting not only the elimination of prolapse but also the restoration of physical activity and psycho-emotional well-being. Complications were rare, did not require re-intervention, and no recurrences were observed during the follow-up period. Avoidance of synthetic materials prevented specific material-related complications while maintaining high durability and functional integrity of the reconstruction. The results confirm that the proposed method is a promising, safe, and effective surgical option for the correction of this pathology.
