GENETIC FACTORS AND THE EFFECTIVENESS OF REHABILITATION AFTER TOTAL HIP ARTHROPLASTY

This paper presents the results of a study on the role of the COL1A1 gene polymorphism (rs1800012) in optimizing rehabilitation after total hip arthroplasty (THA). The prospective study included 115 patients aged 40–80 years, divided into a main group (n=57, personalized rehabilitation) and a comparison group (n=58, standard rehabilitation). Genotyping was performed using real-time PCR, clinical and functional parameters were assessed using the Harris Hip Score (HHS), Timed Up and Go (TUG), 6-minute walk test (6MWT), and visual analogue pain scale (VAS) before surgery, 1, 3, 6, and 12 months after it. Patients with the TT genotype had elevated β-CTX levels (0.65 ± 0.12 ng/ml vs. 0.45 ± 0.09 ng/ml in GG, p<0.05) and slower functional recovery (HHS at 6 months: 82.3 ± 7.1 vs. 88.5 ± 6.2, p<0.05). A personalized program that takes into account the genetic profile and biochemical markers (25(OH)D, osteocalcin) reduced the recovery time by 20% and improved the HHS by 10 points (p<0.01). The results emphasize the importance of genetic testing for individualization of rehabilitation after THA, opening up prospects for predictive orthopedics.
