SURGICAL TACTICS FOR “DIFFICULT” DUODENAL ULCERS

Resume

The authors, as a result of a study of 262 patients with “difficult” DU, in elective UD surgery, came to the conclusion that GR with GDA according to Gaberer is justified in case of bulbous callous and giant ulcers of the anterior wall of the duodenal bulb. As a result of the work performed, modifications of the first Billroth method in the main group of patients were performed in 79.1% of cases, which was one of the measures in the surgical prevention of duodenal stump suture failure. In the absence of the possibility of performing GR in modifications of the first Billroth method, GR was performed according to Hofmeister-Finsterer or Balfour. In case of “difficult” postbulbar DU in combination with CDDP, the operation of choice was GR according to Roux-Ibadov.
