REHABILITATION POTENTIAL IN PATIENTS AFTER SURGICAL REMOVAL OF INTRACRANIAL MENINGIOMAS

Resume
Aim: To evaluate the role of tumor localization, volume, and peritumoral edema in determining the rehabilitation potential of patients following surgical removal of intracranial meningiomas. Materials and Methods: The study included 118 patients with histologically confirmed meningiomas (WHO Grade I-III) who underwent surgical resection. Functional recovery was assessed 3 months postoperatively using the Functional Independence Measure (FIM) and the Barthel Index; cognitive functions were evaluated with the Montreal Cognitive Assessment (MoCA) and the Frontal Assessment Battery (FAB). Results: The severity of peritumoral edema was quantified using the edema index (EI), derived from MRI. Tumors located at the skull base were associated with the most pronounced decrease in cognitive recovery (MoCA 23.1 ± 2.7; p < 0.001) compared with convexity meningiomas (26.2 ± 2.0). Larger tumor volume (≥ 30 cm³) and marked peritumoral edema (EI > 2.5) significantly reduced both functional independence (FIM 98.8 ± 11.5; p < 0.001) and cognitive recovery (MoCA 22.1 ± 2.7; p < 0.001). Longitudinal assessment demonstrated a transient decrease in cognitive performance on postoperative day 10, followed by recovery at 3 months (p < 0.001). The edema index was identified as the strongest predictor of low rehabilitation potential (r = -0.46; p < 0.001).
