Immunological infertility is a significant cause of reduced reproductive function in both men and women. This study investigated the effects of anti-sperm antibodies (ASA) and cytokines on sperm function in men and reproductive structures in women in a cohort of 180 patients. Participants were divided into three groups: the main group (elevated ASA levels), the comparison group (increased levels of specific cytokines), and the control group (healthy individuals). Results demonstrated that high ASA levels in men were associated with reduced sperm motility, viability, and proportion of morphologically normal sperm, as well as increased DNA fragmentation. In women, elevated ASA and imbalanced cytokines negatively affected endometrial function, implantation rates, and pregnancy outcomes. Correlation analysis revealed an inverse relationship between immunological factors and reproductive function in both sexes. Comprehensive assessment of ASA and cytokine profiles provides high diagnostic accuracy and enables individualized prediction and management of infertility. These findings have practical significance for personalized immunological diagnostics, treatment with immunomodulators, antioxidants, and the use of assisted reproductive technologies (ART).
