Chemotherapeutic agents are essential in the treatment of malignant diseases, yet nephrotoxicity remains one of their most severe and frequent complications. Renal dysfunction leads to impaired drug elimination, increased systemic toxicity, treatment interruption, and overall worse prognosis. Platinum-based compounds, alkylating drugs, antimetabolites, targeted therapies, and immunotherapeutic agents are the most nephrotoxic groups. Clinical evidence shows that 30–50% of patients undergoing polychemotherapy develop acute or chronic kidney injury. This review summarizes the mechanisms of renal damage, risk factors, clinical manifestations, consequences on chemotherapy effectiveness, early biomarkers, and current preventive strategies. A deeper understanding of chemotherapy-induced nephrotoxicity is essential for improving treatment safety and preserving renal function in oncology patients.
