FEATURES OF BLOOD PRESSURE VARIABILITY DURING CKD

Resume
The article presents the results of observation of 217 patients aged 18 to 65 years, patients with urinary organs, with stable forms of coronary heart disease and hypertension, endocrine diseases and diabetes mellitus. The survey was conducted in 3 stages. At stage 1, all patients underwent a comprehensive diagnosis of nephropathy, identification of risk factors, the presence of hypertension, blood pressure variability and its relationship with kidney changes. At stage 2 (after 90 days) and stage 3 (after 180 days), a clinical and laboratory examination was performed. Renal filtration function was assessed by endogenous creatinine clearance with calculation of glomerular filtration rate (GFR) using CKD-EPI (2011) formulas for serum creatinine: GFR (ml/min/1.73 m2) = 175 x (serum creatinine, mmol/L / 88.4)-1.154 x (age, years)-0.203, for women the result was multiplied by 0.742. To study the blood pressure level, it was measured using a tonometer with a dial gauge in a sitting position after a 10-minute rest, the measurement was carried out 3x-fold, which were then recorded with the calculation of their average value. The results obtained indicate that in patients with CKD in the early stages, there is a violation of fluctuations in intravisitive variability, while the most unfavorable variant of VAD was observed in patients with severe stages of CKD (30.63%). Interstitial variability (>6.5 mmHg (SAD)) It was observed in 48.69% of the observed (p<0.001) and may be a prognostically unfavorable extrarenal factor in the progression of CKD.
