Objective. Improving the results of treatment of patients with diabetic foot syndrome with critical lower limb ischemia through the use of an improved ultrasound wound treatment method. Materials and methods. The work is based on the examination and treatment data of 123 patients with critical lower limb ischemia with severe diabetic foot syndrome complicated by purulent wounds who received inpatient treatment at the clinical base of the Bukhara State Medical Institute from 2024 to 2025. Results. In severe diabetic foot syndrome with critical lower limb ischemia, the use of well-known methods of modern endovascular interventions and local wound treatment against the background of diabetic foot syndrome using ultrasonic wound treatment a 25% dimexide solution and an electroactivated solution – And in diabetic foot syndrome, it is a more effective way than using ultrasound treatment of a wound with saline solution or using a 25% dimexide solution, which is manifested in accelerating healing processes and improving intoxication rates by an average of 2-4 days, eliminating cases of high amputation, reducing the number of amputations at the shin level from 18.9% to 7.5%, atypical foot resection from 24.3% to 15.0%, reamputation from 5.4% to zero, suppuration of the postoperative stump from 5.4% to 2.5% of cases compared with the II A subgroup. Conclusions. Due to a decrease in the number of highly traumatic operations, there was an increase in the following low-traumatic operations, such as finger amputation from 32.4% to 47.5%, necrectomy from 21.6% to 30.0%, respectively. Mortality in the application of ultrasound wound treatment with a 25% dimexide solution and electroactivated solution - A was not noted, unlike in previous comparison groups. The average length of bed days was 7.1±0.8 days.
