EVALUATION OF THE FUNCTIONAL STATE OF THE THYROID GLAND IN PATIENTS AFTER TOTAL AND SUBTOTAL THYROIDECTOMY

Resume
The extent of thyroid surgery (total vs. subtotal thyroidectomy) affects the subsequent functional state of the gland and the need for replacement therapy. Objective: To compare the frequency and nature of thyroid dysfunction in the early and late periods in patients who underwent total and subtotal thyroidectomy. A retrospective analysis of clinical, laboratory, and instrumental data of patients operated on in the period 2015–2024 (n = 184) was conducted. The levels of TSH, free T4 (FT4), T3, and TPO antibodies, clinical manifestations of hypo- and hyperthyroidism, as well as the prescription of replacement therapy were assessed. Comparison of groups by the extent of surgery (total - TT, subtotal - CTT) was performed statistically (χ², t-test, logistic regression). The risk of developing clinical hypothyroidism is statistically higher in the subgroup with STT and concomitant autoimmune thyroiditis and a smaller volume of residual tissue.
