MATERNAL RISK FACTORS FOR THE DEVELOPMENT OF ANTENATAL FETAL DEATH

According to the World Health Organization (WHO), fetal death is defined as the death of a fetus prior to its complete expulsion or extraction from the mother, regardless of the duration of pregnancy. The term “stillbirth” is applied only to pregnancies of 28 weeks of gestation or more. Significant progress in perinatal medicine has resulted in a substantial reduction in the rate of stillbirths, particularly in developed countries; currently, approximately 98% of stillbirths occur in low-income and developing countries. Stillbirth has a profound psychological and medical impact on both the patient and the physician. Since nearly half of stillbirth cases occur in apparently uncomplicated pregnancies, it is critically important to examine known predictors of intrauterine fetal death. Among preterm and term fetuses, perinatal mortality is higher in those who are small for gestational age, with the risk increasing proportionally to the severity of fetal growth restriction. Prolongation of the first stage of labor is not associated with an increased risk of perinatal mortality or morbidity; however, a prolonged second stage of labor is associated with increased mortality and neonatal morbidity, including sepsis, seizures, and hypoxic-ischemic encephalopathy. Ultrasound examination of the placenta and umbilical cord is a mandatory component of pregnancy monitoring. Various ultrasound findings have been associated with adverse perinatal outcomes, including intrauterine fetal death. After reviewing the evidence regarding predictors of intrauterine fetal death, we propose a general strategy aimed at reducing the likelihood of stillbirth.
