ANALYSIS OF IMMEDIATE AND LONG-TERM TREATMENT OUTCOMES IN PATIENTS WITH INFECTED PANCREATIC NECROSIS AGAINST THE BACKGROUND OF DIABETES MELLITUS

This scientific article analyzes the immediate and long-term outcomes of surgical treatment in patients with infected pancreatic necrosis (IPN) on the background of diabetes mellitus (DM), depending on the surgical techniques applied. A total of 188 patients with IPN, treated between 2012 and 2025, were examined. The control group consisted of 62 patients (33%) with IPN and concomitant DM, treated between 2012 and 2018 using a traditional approach. The comparative group included 62 patients (33%) with IPN without DM. The main group comprised 64 patients (34%) with IPN and concomitant DM, treated from 2019 to 2025, in whom the developed therapeutic-diagnostic algorithm was applied, based on a structured assessment of disease severity and a stepwise selection of surgical tactics. Immediate treatment outcomes were evaluated using the Clavien–Dindo classification. Long-term outcomes were assessed up to 12 months after hospital discharge. Overall, the study results demonstrated a reduction in the frequency of local and systemic complications, as well as decreased mortality, compared to the traditional treatment approach. A reduction in the frequency of late surgical complications was also observed with the differentiated and stepwise approach during long-term follow-up.
