DIFFERENTIATED TREATMENT APPROACH TO INFERTILITY IN POLYCYSTIC OVARY SYNDROME AND ITS IMPACT ON FERTILITY OUTCOMES

Resume

Polycystic ovary syndrome (PCOS) is a leading cause of endocrine infertility in women of reproductive age. Aim: To assess fertility restoration depending on treatment strategy and infertility duration in women with PCOS. Material and Methods: A prospective study included 70 infertile women with PCOS and 20 healthy controls. Patients were divided into three subgroups: conservative therapy with combined oral contraceptives (COCs), combined therapy with COCs followed by laparoscopic ovarian drilling, and surgical treatment alone. Hormonal, metabolic, and ultrasound parameters were evaluated. Results: The combined approach demonstrated the most significant normalization of hormonal profiles and the highest pregnancy and live birth rates. Conclusion: Stepwise combined treatment provides superior and sustainable fertility restoration compared to isolated surgical intervention.
