Epilepsy is one of the most common chronic disorders of the central nervous system and is characterized by recurrent epileptic seizures. In modern epileptology, epilepsy is regarded not only as a paroxysmal condition but also as a complex neuropsychiatric syndrome accompanied by cognitive and psychoemotional impairments. This article presents a review of current literature data on the mechanisms of cognitive dysfunction in epilepsy, its clinical manifestations, and pathogenetic factors. The association of cognitive impairment with age at disease onset, epilepsy type, localization of the epileptogenic focus, interictal epileptiform activity, and antiepileptic therapy is analyzed. The reviewed data emphasize the importance of early detection of cognitive disorders and the implementation of a comprehensive therapeutic approach in patients with epilepsy.
