Rheumatoid arthritis is a chronic autoimmune disease characterized by systemic inflammation and impaired bone metabolism, which may adversely affect the osseointegration of dental implants. This prospective comparative study (2023–2025, n=80) aimed to evaluate clinical, laboratory, and radiological factors influencing dental implant osseointegration in patients with rheumatoid arthritis. The study included patients with rheumatoid arthritis who underwent dental implantation (n=28), systemically healthy individuals with dental implants (n=27), and patients with rheumatoid arthritis without implant treatment (n=25). Bone tissue status was assessed using dual-energy X-ray absorptiometry and cone-beam computed tomography with measurement of alveolar bone density in Hounsfield units; systemic inflammatory markers (CRP, ESR) and cytokines (IL-6, IL-10, TNF-α) were additionally evaluated. Patients with rheumatoid arthritis demonstrated a statistically significant reduction in bone mineral density and alveolar bone density compared with the control group (p<0.05). A strong positive correlation was found between bone density and primary implant stability parameters (r=0.68; p<0.001). An alveolar bone density threshold of ≥678 HU was associated with a favorable prognosis for osseointegration. The overall success rate of dental implantation in patients with rheumatoid arthritis was 96.7%, comparable to that in healthy individuals. A comprehensive diagnostic algorithm incorporating clinical and laboratory assessment of disease activity and CBCT-based evaluation of alveolar bone density was proposed, improving the safety and predictability of implant treatment in patients with rheumatoid arthritis.
