After birth, the infant enters an open system filled with microorganisms, foreign proteins, and chemicals, lacking even the placental protection. However, during this period, the mammary gland and maternal milk become of primary importance for the newborn. This article presents a literature review on the various bioactive components of maternal milk that influence the immune status of infants, not only providing protection but also facilitating its development, tolerance, and the corresponding inflammatory response. It can be hypothesized that milk acts as a medium of communication between the maternal and infant immune systems, actively guiding and training the infant's immune, metabolic, and microbial systems, while providing multiple defenses against pathogens. The physiological and protective functions of many of the immune components of breast milk were based not on studies in infants, but on what is known in other species and in vitro models. This update briefly reviews the development of the infant immune system, focusing on current knowledge of how “classical” immune and non-immune components contained in mature breast milk may promote immune development, facilitate tolerance, and regulate the infant's inflammatory response.
