EARLY PREDICTION OF PREECLAMPSIA IN PREGNANCY VIA IN VITRO FERTILIZATION USING BIOMARKERS AND DOPPLEROGRAPHY

Rationale. Pregnancies achieved through assisted reproductive technologies, especially in vitro fertilization (IVF), are characterized by an increased risk of developing hypertensive complications, including preeclampsia. Early identification of women at high risk is crucial in reducing maternal and perinatal complications. Objective. To assess the diagnostic value of clinical, biochemical, and Doppler parameters in predicting preeclampsia in women who became pregnant through IVF. Materials and methods. A prospective cohort study was conducted in the Bukhara city maternity complex from 2023–2025. Eighty women who became pregnant through IVF were included in the study: the main group consisted of women with preeclampsia (n=48), and the control group consisted of women with uncomplicated pregnancies (n=32). The levels of lactate dehydrogenase (LDH), homocysteine, C-reactive protein (CRP), vitamin D, and D-dimer were determined in the blood. Pulsation and resistance indices were assessed by Dopplerography in the uterine arteries. Correlation, binary logistic regression, and odds ratio (OR) were calculated in the statistical analysis. Results. In women with preeclampsia, LDH, homocysteine, CRP, and D-dimer levels were significantly higher, while vitamin D levels were lower (p<0.05). Doppler examination revealed increased vascular resistance in the uterine arteries. Multivariate analysis confirmed homocysteine, LDH, and CRP as independent prognostic factors for preeclampsia. Conclusion. A comprehensive assessment of biochemical markers and uterine artery Doppler parameters allows for early prediction of preeclampsia in IVF pregnancies and the formation of a high-risk group.
