Purpose of the study: Optimization of pregnancy management tactics in women with chronic arterial hypertension, based on the identification of hemodynamic, clinical, and laboratory markers for early preclinical diagnosis of preeclampsia layering. Materials and methods of research: The studies were conducted starting from the first trimester of pregnancy on a contingent of 100 women with anamnestic data on the risk of developing PE. In all women, indicators of systemic hemodynamics, central hemodynamics, and regional analysis using the Doppler method were studied; a clinic for biochemical research was utilized, and glomerular filtration rate indicators were calculated. Results of the study: The blood flow in the renal artery of the SD was 2.62, which was 18.5% more than in the control group. Respectively, impaired renal blood flow affected the parameters of clinical and laboratory data. Cystatin-C increased to 1.05 ± 0.30 compared with the data of the control group (0.55 ± 0.15), an increase of 52.3%. When comparing three markers of GFR (urea, creatinine, and cystatin), Cystatin-C showed diagnostic value as a marker predicting impaired renal function due to a decrease in glomerular filtration rate, which was associated with impaired organ blood flow in the renal vessels. Therefore, serum cystatin-C closely reflects the functional changes in the kidneys, which lead to an increase in blood pressure and urinary albumin excretion as a marker of the stage of transition from CAH to preeclampsia.
