OPTIMIZING THE DIAGNOSIS AND SURGICAL TREATMENT OF INTESTINAL OBSTRUCTION IN CHILDREN

Resume

Intestinal obstruction in children remains one of the leading causes of emergency surgical interventions and is associated with a high risk of complications if diagnosed late. The aim of the study was to evaluate the effectiveness of an optimized algorithm for the diagnosis and surgical treatment of intestinal obstruction in children. We analyzed the treatment outcomes of 112 patients aged 6 months to 17 years, hospitalized in 2021–2024. Patients were divided into two groups: a study group (n=63), which utilized the developed algorithm, and a control group (n=59), which received conventional treatment. The algorithm included a standardized diagnostic approach with a focus on ultrasound, clarifying tomography when necessary, and the preferential use of laparoscopic technologies. Implementation of the algorithm increased the frequency of laparoscopic interventions (88% versus 20%), reduced the incidence of postoperative complications (5% versus 18%), halved blood loss, and shortened the duration of surgery (45±12 minutes versus 70±18 minutes). The average hospital stay decreased from 8±1.5 to 5±1.2 days, and the need for repeat interventions decreased from 7% to 2%. There were no fatalities in the study group. The results indicate that the algorithmic approach ensures earlier diagnosis, reduces surgical trauma, and improves clinical outcomes. The developed algorithm can be recommended for widespread implementation in pediatric surgical hospitals.
