Resume

Thoracoabdominal injuries involve combined trauma to the thoracic and abdominal cavities, characterized by obligatory injury to the diaphragm and peridiaphragmatic structures. These injuries are distinguished by a severe clinical course, a high frequency of diagnostic errors, and a significant mortality rate, which can reach up to 90% in certain injury patterns. The primary challenge lies in the low efficacy of preoperative diagnosis; the sensitivity of traditional imaging modalities remains insufficient, leading to delayed detection of diaphragmatic defects and the development of severe complications, including strangulated diaphragmatic hernias. Currently, there is no consensus on surgical management, and the choice of intervention is often determined by the surgeon's subjective experience, thereby increasing the risk of tactical errors. Contemporary research underscores the importance of a rational selection of surgical approach and the expanded use of minimally invasive technologies—specifically laparoscopy and thoracoscopy—which possess high diagnostic value and facilitate simultaneous therapeutic interventions. In the context of ongoing healthcare reforms in the Republic of Uzbekistan, special attention is being directed toward the development of emergency medical care, the equipping of clinics, and personnel training. These efforts create the necessary prerequisites for the broader implementation of endovisual technologies. The authors emphasize the necessity of developing unified diagnostic and treatment algorithms to reduce complication rates and improve outcomes in patients with thoracoabdominal injuries.
