Combined thoracoabdominal injuries represent one of the most severe forms of trauma and are associated with high mortality and a substantial incidence of postoperative complications. Bronchopulmonary complications—including atelectasis, pneumonia, acute respiratory distress syndrome, and ventilator-associated pneumonia—play a leading role in adverse outcomes. A significant proportion of deaths is related not to injuries incompatible with life but to tactical errors, diagnostic delays, and insufficient prevention of respiratory complications. This narrative review analyzes current concepts of the pathophysiological mechanisms underlying bronchopulmonary complications in combined thoracoabdominal trauma and examines the evolution of surgical tactics and choice of operative approach from a preventive perspective. Particular attention is paid to diaphragmatic injuries, the implementation of minimally invasive techniques, and the role of ventilator-associated pneumonia as a major determinant of postoperative morbidity. Modern strategies for the prevention of bronchopulmonary complications based on the integration of surgical and intensive care approaches and early risk stratification are discussed.
