Early diagnosis of Acute Adhesive Small Bowel Obstruction (ASBO) remains a challenging task, as clinical manifestations at the onset of the disease may be nonspecific, while ischemic changes in the intestinal wall remain occult. Plain abdominal radiography revealed Kloiber’s cups (air-fluid levels) in 83.6% of patients. Contrast-enhanced MSCT was recognized as the most accurate diagnostic method (93–100%). Post-treatment duplex ultrasound scanning revealed statistically significant differences in Vmax and Vmin between patients with localized and extensive adhesions (p<0.001). A comprehensive diagnostic algorithm for ASBO improves the accuracy of early disease detection, helps assess the severity of ischemia, and facilitates the selection of an optimal therapeutic strategy.
