EVALUATION OF THE EFFECTIVENESS OF PLATELET-RICH FIBRIN IN THE SURGICAL TREATMENT OF MEDICATION-RELATED OSTEONECROSIS OF THE JAWS

Resume

Medication-related osteonecrosis of the jaws remains one of the significant problems in oral and maxillofacial surgery, developing as a complication of antiresorptive and antiangiogenic therapy. The aim of this study was to evaluate the effectiveness of platelet-rich fibrin (PRF) in the surgical treatment of this pathology. A retrospective comparative analysis was performed on the treatment outcomes of 49 patients with medication-related osteonecrosis of the jaws who underwent surgical treatment between 2024 and 2026. The patients were divided into two groups: standard surgical treatment (24 patients) and surgical treatment with additional application of PRF (25 patients). The primary outcome measure was complete clinical healing of the mucosa in the surgical area. The results showed that the use of PRF significantly increased treatment effectiveness: the rate of complete clinical healing was 92.0% in the PRF group and 62.5% with the standard surgical approach (p = 0.03). The relative risk of a favorable outcome increased by 1.47 times, the absolute increase in effectiveness was 29.5%, and the number needed to treat (NNT) was approximately 3–4 patients. The obtained data indicate the high clinical effectiveness of local application of platelet-rich fibrin and allow it to be considered a promising component of the surgical treatment of medication-related osteonecrosis of the jaws.
