IMMUNOLOGICAL FEATURE OF AVASCULAR NECROSIS OF THE FEMORAL HEAD IN RHEUMATOLOGY

Clinical, immunological, and metabolic alterations observed in patients with avascular necrosis of the femoral head in the context of rheumatologic diseases reflect the multifactorial nature of its pathogenesis. A high prevalence of concomitant cardiovascular pathology and lipid metabolism disorders has been identified, indicating a significant role of vascular factors in disease development. Elevated levels of proinflammatory cytokines, including tumor necrosis factor alpha, interleukin 6, and interleukin 1 beta, confirm the involvement of immune-inflammatory mechanisms in disease progression. At the same time, baseline immunological parameters remain largely unchanged, suggesting the predominance of functional rather than quantitative immune disturbances. In addition, signs of hypercoagulability, including increased levels of D-dimer and fibrinogen, as well as dyslipidemia, are associated with impaired microcirculation and the development of bone ischemia. Thus, avascular necrosis of the femoral head should be considered a systemic condition resulting from the interaction of vascular, immunological, and metabolic factors.
