The use of fixed orthodontic appliances (FOA) significantly alters the oral homeostasis, often leading to chronic inflammatory diseases of the oral mucosa. Traditional preventive protocols frequently fail to account for individual physiological thresholds, necessitating a more personalized diagnostic approach. Aim: To evaluate the effectiveness of an individualized approach to the prevention and treatment of oral mucosal diseases in patients undergoing orthodontic correction based on clinical and functional monitoring. Materials and Methods: A study was conducted on 120 patients (aged 18–35) divided into a control group (standard care) and a study group (individualized protocol). Diagnostics included laser Doppler flowmetry (LDF) for microcirculation assessment, electrometric pH-metry of oral fluid, and PCR-based microbiological monitoring of periodontal pathogens. Results: The study revealed that FOA placement leads to a significant decrease in the microcirculation efficiency index (by 18.4–22.1%) and a drop in salivary pH (below 6.2) within the first 30 days. The individualized protocol, incorporating targeted antiseptic therapy and microcirculatory support, reduced the recurrence of inflammatory lesions by 38.6%. Furthermore, a strong correlation was found between stabilized pH levels and improved patient quality of life (OHIP-14). Conclusion.Transitioning from standardized to personalized preventive algorithms based on functional markers (LDF and pH monitoring) significantly enhances the safety of orthodontic treatment and prevents iatrogenic damage to the oral mucosa.
