Background: The menopausal transition period is often accompanied by a deterioration in women’s well-being and quality of life. Although the effectiveness of menopausal hormone therapy (MHT) has been confirmed by numerous studies, its utilization among patients remains low. The influence of organizational factors within healthcare systems and the level of medical awareness on women’s refusal of therapy in Central Asian countries has been insufficiently studied. Objective: To assess the medical and social determinants, the level of awareness, and indicators of access to healthcare among women during the menopausal transition. Materials and Methods: A cross-sectional study was conducted involving 451 women aged 46–67 years. A questionnaire was used to evaluate access to healthcare and the quality of communication with physicians. Statistical analysis included Pearson’s χ² test and Fisher’s exact test. Results: The integral index of organizational accessibility was 10.07±1.72 points (median 11), while the communication quality index was 4.74±0.78 points (median 5). Despite well-developed infrastructure, 80.2% of women had never consulted a physician previously. The main barrier was hormonophobia (36.4% of respondents). Only 36.0% had sufficient knowledge about MHT, and awareness was significantly associated with healthcare-seeking behavior (p < 0.001).
